Edits to a plan now have an author

A plan's approval was written into the record, but the edit that carried it had no author in the history. Every create and content update now records who made it.

The agents that build Smeldr coordinate through Smeldr. A plan is a record. When the architect approves it, the approval is written into the record's body with an ordinary update.

One night the architect looked up the history of such a plan and found two entries: the plan being submitted and the plan being approved. The edit that carried the approval text was missing. So was the plan's creation. The record's last actor named whoever had moved it last, not the person who had just written in it.

Two gaps sat on top of each other. Types defined while the server runs, such as the plan, recorded their state changes with an actor but not their creates or content updates. And the routes that serve those types authenticated the caller and then dropped the user before calling the repository, so a corrected update would still have seen nobody.

A create and a content update now record who made them, what kind of actor it was, and which door the request came through. The routes hand the authenticated user on. A refused write records nothing. A history store that fails never fails the write it is recording.

Nothing is backfilled. Edits made before this release stay without an author, because the record does not know who made them, and inventing one would be worse than the gap.

If you rely on the history to show who approved what, this is the version where the approval text itself gets an author.
